Kamva Medical operates two clinics in London, Ontario:

2. Our Privacy Promise

We follow Ontario’s Personal Health Information Protection Act (PHIPA). That means we collect and use your health and personal information only to give good medical care. We keep it safe and share it only when needed for care or when required by law.

3. What Information We Collect

• Personal details: name, date of birth, contact info, health card number

• Health info: medical history, diagnoses, treatments, lab results

• Appointments: dates you visit, service notes, billing info

We collect info when you register, visit, fill in forms, or talk with our staff.

4. How We Use Your Information

We use your personal health information to:

• Provide medical and follow‑up care

• Communicate appointment reminders or test results

• Bill Ontario Health or insurance for services

• Share with other doctors or hospitals if you’re referred or transferred

• Improve how the clinic works

5. How We Keep It Safe

• Staff must only access patient info they need

• We use secure clinics and computer systems

• Regular training for staff on privacy rules

• Paper files stored securely; digital systems encrypted

6. When We Might Share Info

• To other healthcare providers involved in your care

• If the law requires (e.g. public health, court order)

• For emergencies when you cannot give consent

We do not sell or trade personal health info.

7. Keeping Your Info Correct

You can ask us to see your record or ask for changes if it’s wrong. This is called access or correction under PHIPA.

8. Consent and Any Limits You Give

You can tell us how your info is used or shared.
You may ask us to limit who sees parts of your record (this is called a “lockbox” or consent directive). But note: that might limit care if doctors can’t see details they need.

9. If Things Go Wrong: Privacy Breach

If there’s ever a privacy incident (like a lost laptop or unauthorized access), we will:

1. Tell you right away if it could cause serious harm

2. Investigate and fix the issue

3. Report to Ontario’s Information and Privacy Commissioner if needed

10. How Long We Keep Records

We keep records according to Ontario law. Usually, we keep health records for many years—some up to 10 years or more.

11. Your Rights

• Ask to see or correct your record

• Ask questions about our clinic’s privacy rules

• Ask to limit who sees your info or how we use it
